COVID-19 Testing Consent Form Available in NYCSA

As noted in the September 30 edition of Principals Digest, you should distribute and collect student COVID-19 testing forms from families. In addition to the paper form, a digital version is available via NYC Schools Account (NYCSA). As families complete the information in NYCSA, their responses are displayed in the student indicator value ‘COVID TESTING’ for individual students on the Biographical Individual Student Indicator Display (BIND) function in ATS. Translations of the form will be available in all DOE-supported languages via NYCSA on October 6. For additional information on COVID-19 testing, please visit the COVID-19 Testing for Students wiki.
